Financing Options
Payment Options - For your convenience, we accept cash, personal checks, Heath Savings Accounts (HSA), money orders and major credit cards. Payment is expected at the time services are performed. When more extensive dental care is necessary, financial arrangements can be made with our office.
Care Credit Financing: 6 months or 12 months. No finance charges if paid in full prior to the selected frame.
Insurance - We are a Preferred Provided for Ameritas and Delta Dental (Washington Dental Service), As a service to our patients, our practice accepts most dental insurance programs, including non-managed care, indemnity (traditional) and PPO out-of-network. We are not part of any managed care network. Our accounting staff will prepare all the necessary forms for your dental benefits. However, we remind you that your specific policy is an agreement between you and your insurance company. Please keep in mind that you are responsible for your total obligation should your insurance benefits result in less coverage than anticipated. Our staff will gladly submit a pre-treatment estimate to your insurance company so that you will know what your benefits will be.
The fees charged for services rendered to those who are insured are the usual and customary fees charged to all our patients for similar services. Your policy may base its allowances on a fixed fee schedule, which may or may not coincide with our usual fees. You should be aware that different insurance companies vary greatly in the types of coverage available. Also, some companies take care of claims promptly while others delay payment for several months.
FINANCIAL AGREEMENT
You are responsible for the cost of treatment provided in our office. Please take time to know your coverage and limitations. Payment is due at time services are rendered.
• Total estimated cost of treatment will be given to you after your new patient exam. This is an estimate not a guarantee of insurance coverage
• At the time of service, 50% of the patient’s portion of the procedure cost is due; the remaining balance is due within two months of service date.
• We accept cash, checks, Discover®, VISA®, MasterCard®, American Express® and CareCredit®
• We offer a 5% discount to patients who pay their bill IN FULL at time of service with cash or check and are not using insurance.
Insurance Coverage
We are preferred providers for Ameritas and Delta Dental/WDS and will also bill all insurance companies. Treatment is based on your dental health needs, not your insurance coverage. You are responsible for full payment if the insurance company doesn't pay. Patient’s portion is due at the time of service. If the insurance payment is not received within 60 days, you will need to contact your insurance company.
Finance Fees
It is our policy to charge a monthly finance fee at 1.5% for outstanding patient balances after the balance has been outstanding 60 days. In addition, all payments returned due to non-sufficient funds will be subject to a NSF fee of $25.00.
Medicare
Dr. Cardenas’ services are not covered under Medicare/Medicaid and no Medicare/Medicaid payment may be made to any entity for her services, directly or indirectly.
Appointment Cancellation
Unless cancelled at least 48 hours in advance, our policy is to charge for missed surgery appointments at the rate of $500 per appointment time. Please help us service you better by keeping scheduled appointments.
